If registering a child(ren) for MOPPETS, fill out and return this form.
Dues for fall 2012*:
$24 for MOPS International
$25 local dues
Total: $49.00
Checks may be made payable to Covenant Baptist Church, with "MOPS" in the memo line.
Dues for spring 2013 (payable at the beginning of 2nd semester):
